Customization Order Form & Terms
Customer: Town of Avon
Pricing: The estimated cost for customization of Economic Impact Data Input Validation and Economic Impact Reports
for statistical sampling information is as follows. As noted, MUNIRevs will discount our actual cost by 50% to share in
the cost of this new code that will be added to the MUNIRevs product(s) for possible use by other jurisdictions.
• Data Validation Input Customization - $2,000
• Move the Certification language below the Event Data input, rather than where it currently resides
which is above the event data and below the tax form.
• Disallow input of event gross sales that is > tax form gross sales for the same period
• Disallow input of event gross sales that is < 1% of tax form gross sales for the same period. If input is <
1 %, taxpayer will be allowed to SAVE their form but not to SUBMIT their tax form to complete and
submit the required amount due to the Town. Town of Avon to provide MUNIRevs with the language
desired in Pop Up Message that will display in this scenario.
■ Pop Up Message will include specific contact information for a Town of Avon staff member
rather than MUNIRevs Support.
• Economic Impact Event Report Customization —
o On line item details for each vendor's data:
• Inclusion of vendor's gross sales for the period of the event
• Calculation of the vendor's sales and accommodation taxes (if applicable) that the Town of Avon
receives based upon the vendor's gross sales from the event.
■ Inclusion of percentage that event gross sales were in comparison to the period of the event.
o On Event Summaries - $4,000
• Inclusion of Gross Sales for all sampled vendors
• Display of sales and accommodation taxes collected by the Town of Avon based upon Sample
Set Gross Sales for event period
■ Display of Gross Sales for all vendors in the same NAICS and Business Area grouping
■ Display of Percentage of sample set vs. the same NAICS and Business Area Grouping
■ Display of all gross sales for the period for the Town of Avon
■ Display Percentage of sample set vs. all sales for the Town for the Period
• Discount — 50% Cost Sharing between MUNIRevs and Town of Avon. Net Cost: ($3,000)
• Other —
• MUNIRevs will assist Town of Avon in deploying Info Messages to business owners with the applicable
Economic Impact Settings to increase participation.
• In addition MUNIRevs will continue to assist Town of Avon in using the MUNIRevs reports to begin their
Constant Contact (or other e-mail outreach system) updates for communications with vendors about
non -tax or licensing issues outside the MUNIRevs system.
■ NOTE: MUNIRevs recommends that any non -tax or licensing communications, using the contact
data available through MUNIRevs, occurs outside the MUNIRevs system so as to not detract
from the collection and tax compliance nature of MUNIRevs system communications.
• Net Cost to Town of Avon - $3,000
Terms: This order is a part of and governed by the terms of the Master Subscription Services Agreement between the
parties (collectively "Agreement "), which terms are incorporated into this order for all purposes. The Term of the
Agreement shall renew with this order unless otherwise terminated in accordance with the terms of the Agreement.
Monthly Fees: There are no additional monthly fees for this customization. I
Due Dates: n
• MUNIRevs to deliver Order Form (This Document) to Town of Avon for Review by 7/23/2014
• MUNIRevs commence work on the above upon receipt of a signed order form from the Town of Avon.
• MUNIRevs will deliver above work product for review & approval by Town of Avon within 30 days of receipt of
Order Form.
• Payment is due upon approval of completed work by Town of Avon staff.
