Avon Mall Final Design Amendment
This Task Order pertains to the On-Call Agreement by and between the Town of Avon
(“TOWN”), and Stolfus and Associates (“CONTRACTOR”), dated On-Call Contract Execution
of September 3, 2021 (“the Agreement”).
CONTRACTOR shall perform services as provided herein and in the Agreement. This Task
Order shall not be binding until it has been properly signed by both parties. Upon
execution, this Task Order shall supplement the Agreement as it pertains to the project
described below.
Project Description: Provide additional design scope of work.
• Subsurface Utility Engineering – includes 10 test holes
• Drainage design: recreation center parking lot, analysis of swale and old Fire station,
pipe connecting two rain gardens
• Structure Design: retaining wall east of Mikaela Way, foundation for flagpoles and
string lights, and seat wall foundations
• Lighting/Electrical: power for food trucks, uplighting for shade sails, parking lot
lights
• Roadway: recreation center parking lot shift west, widened sidewalk in front of
recreation center
• Landscaping: additional landscaping areas, updated exhibits for Town meetings

Dear Eva,
The Town of Avon has identified a need to extend the Avon Main Street Mall from Lettuce Shed Ln to Nottingham
Park. This task order adds the following services for final design:
• Subsurface Utility Engineering – includes 10 test holes
• Drainage design: recreation center parking lot, analysis of swale and old Fire station, pipe connecting two
rain gardens
• Structure Design: retaining wall east of Mikaela Way, foundation for flagpoles and string lights, and seat
wall foundations
• Lighting/Electrical: power for food trucks, uplighting for shade sails, parking lot lights
• Roadway: recreation center parking lot shift west, widened sidewalk in front of recreation center
• Landscaping: additional landscaping areas, updated exhibits for Town meetings
SCOPE OF WORK
Work Element 1 – Project Administration / Coordination (Stolfus & Associates, Inc.)
Stolfus & Associates, Inc. (Stolfus), will review/process invoices, as well as provide other project administration/
management services (coordination and product review) throughout the duration of the project. Stolfus will be
responsible for day-to-day management and coordination of the work elements included in this scope of work.
Stolfus will work with the Town’s project manager to modify schedule of activities for each task element, as
appropriate.
Work Element 1 Deliverables:
• Schedule
• Monthly invoices and progress reports
Work Element 2 – Subsurface Utility Engineering (Farnsworth Group)
Farnsworth Group will provide SUE investigations. See attached.
Work Element 3 – Path Alignment/Grading & Roadway/Traffic/Parking Lot Design (Stolfus)
100% Design
Based on 30% comments, Stolfus will realign the path between Lettuce Shed Ln and Mikaela Way. Stolfus will
shift the recreation center parking lot west to accommodate the widened sidewalk in front of the recreation center
that goes north to Lake St. Horizontal and vertical alignments will be created for the sidewalk connection as well
as a 3-D model. The parking lot will be designed and a 3-D model will be developed to understand the grading
impacts and achieve positive drainage. Stolfus will modify the grading at the rain gardens to enhance drainage
flow between the two rain gardens; a pipe under the Seasons access across from Town Hall will be investigated.
If a pipe is not feasible, a gutter pan will be designed.

Stolfus will add the following sheets to the 100% plans:
• typical sections,
• geometric control plans,
• summary of approximate quantities,
• tabulations,
• removal plans (parking lot),
• horizontal roadway/path plans (additional parking lot plan and sidewalk),
• grading plans (with spot elevations for parking lot),
• urban design and landscape plans (see Work Element (WE) 4)
• lighting plans (including additional lights and food truck power) (see WE 6)
• structural details for wall, flagpole & light pole foundations (see WE 7)
Prior to submittal to the Town of Avon, Stolfus will perform a quality control (QC) review of deliverables for
completeness, accuracy, and appropriateness. The Stolfus Project Manager will be responsible for scheduling QC
reviews with senior technical staff.
The Town will conduct a review of the 100% documents and provide written comments to the Stolfus Team for
inclusion in the Ad set. Stolfus will provide written responses to comments from the 100% design.
Work Element 3 Deliverables:
• 100% plans (additional sheets)
Work Element 4 – Urban/Landscape Design (DHM Design)
DHM will work closely with Stolfus and the Town to develop detailed construction plans and specifications for
contractor bidding. The following services are added to the project:
• Provide added paver walkway extending north from the pedestrian mall to Lake St. This would include an
additional layout, landscape, and irrigation plan sheet.
• Provide additional flagpoles west of Lake St.
• Modify roll plot and render for CASE and DDA meetings
All deliverable products remain the same as the original contract. This proposal does not include 3D renderings.
Work Element 4 Deliverables:
• One (1) Roll Plot update for use by Town in CASE and DDA meetings
• 100% plans, specifications and OPC
Work Element 5 – Drainage and Water Quality (RESPEC)
RESPEC will prepare calculations and plans for the drainage improvement listed below. Plans will be prepared to
a 100% bid level. Quantities and costs will be prepared. An abbreviated drainage memo will also be provided
• New Inlet and pipe at the west end of the modified recreation center parking lot
• Analyze and possibly modify drainage at the old Fire Station.
• Replace inlets at Lake Street and Mikaela Way.
• Rain garden grading and drainage review and minor modifications.
One additional site visit will be conducted.

Work Element 6 – Lighting and Electrical (Clanton)
Clanton will provide lighting and electrical design services for the following elements:
1. Food Truck Power
• Provide power for two (2) Food trucks at the Lake St. Plaza
2. Provide lighting and electrical design for up lights on shade sails at Lake Street Plaza
3. Rec Center parking lot lighting
• Provide lighting and electrical design for relocated rec center parking lot
• Provie new Bollard lighting and electrical design for bollards in front of Rec center
4. Provide Holiday Receptacle on all bollards along the Pedestrian Mall Walk
5. Up to three (3) coordination meetings
It is assumed that the parking lot lighting and bollards will match the overhead Roadway lights and bollard.
Work Element 6 Deliverables:
• 100% plans (modified to include above)
Work Element 7 – Structural Support (San Engineering)
San Engineering will provide structural design support for the design of the retaining wall east of Mikaela Way and
miscellaneous stie structures including flagpole foundations, string light pole foundations, and seat wall
foundations as identified during the 30% design review meeting. San will provide structural calculations and
structural details of the wall east of Mikaela Way for inclusion in the construction documents. Plan and profile
view of the wall is not included.
Miscellaneous Site Structures include:
• Flagpole foundations (3 total)
• String Light Pole Foundations – Various conditions
o South of pedestrian wall only. It is assumed the north will connect to future Lot B development.
• Seat Wall Foundations (2 total)
Updates and comments will be addressed at 100% for inclusion in the Contract Documents.
Work Element 7 Deliverables:
• 100% details for wall east of Mikaela St
• 100% details for miscellaneous structures: seat walls/flagpole/string light pole foundations

As detailed below, Stolfus and its subconsultants will perform these engineering services for a total fee of
$127,315.
