On-Call Agreement for Professional Services
TASK ORDER 1-2024-3
Avon Road Rapid Rectangular Flashing Beacon Design
This Task Order pertains to the On-Call Agreement by and between the Town of Avon
(“TOWN”), and Stolfus and Associates (“CONTRACTOR”), dated On-Call Contract Execution
of September 3, 2021 (“the Agreement”).
CONTRACTOR shall perform services as provided herein and in the Agreement. This Task
Order shall not be binding until it has been properly signed by both parties. Upon
execution, this Task Order shall supplement the Agreement as it pertains to the project
described below.
Project Description: Avon Road traverses north-south through the center of Avon, CO.
The CONTRACTOR and the Town have identified Nine (9) pedestrian crossings on or
adjacent to Avon Road to be upgraded with flashing beacon pedestrian crossing signs.
Further, these upgrades require ADA compliant curb ramps for accessibility. Six (6) of the
crossings are located on CDOT maintained infrastructure, while three (3) of the crossing
including ten (10) curb ramps are part of the Town of Avon infrastructure. The
CONTRACTOR shall obtain necessary Field Survey and develop a Final Design Plans,
Specifications, and Estimate package for the installation of the 10 Avon curb ramps in
compliance with Public Right-of-Way Accessibility Guidelines (PROWAG).

Dear Nicole:
Please find below the scope of work for design services for the Avon Road Curb Ramps project to
design 10 curb ramps along Avon Road. Improvements include the following:
· Beaver Creek Blvd – South Crossing – East, West, and Median Curb Ramps
· Benchmark Place – South Crossing – East, West, and Median Curb Ramps
· Hurd Lane – South Crossing – East and West Curb Ramp
The following describes the proposed scope of services:
· Horizontal layout and vertical point design / detail of curb ramps
· Opinion of Probable Cost (OPC) for curb ramps
It is assumed that this project will be its own plan package that will not need approval through the
CDOT Local Agency process. If the project is combined with the Avon RRFB project as one plan
package, it is assumed that these elements will get approved through the RRFB project process. No
additional time to meet CDOT Local Agency project requirements is included in this scope of work.
ELEMENTS OF THE SCOPE OF SERVICES
A. Project Administration/Management
Stolfus will review and process invoices and will provide other project administration/ management
services (coordination and product review) throughout the duration of the project. We will also be
responsible for day-to-day management and coordination of the work elements.
Stolfus will assist the Avon Project Manager with the maintenance of the design schedule and
coordination with multi-disciplinary team members. In addition, two major project meetings are included
in this scope. It is assumed that both major project meetings will be virtual. Meeting minutes will be
provided for major meetings.
Meetings:
· 1 – Kickoff Meeting with Town (Virtual)
· 1 –60% Review meeting with Town (Virtual)
· Monthly Check Ins (Stolfus and Town of Avon) (Assumes 6)
B. Curb Ramp Design
Stolfus will be responsible for the design of the curb ramps. It is anticipated that the design will include
the following elements:

· Vertical Point Design

Point tables will be generated for the curb ramp details to depict elevations at key points within the curb
ramp. Point tables will depict northing, easting, and elevation data. Stationing/alignments will not be
generated for this project. Curb ramp locations will be coordinated with the Town of Avon’s RRFB
project.
C. 60% Plans and Specifications
Stolfus will provide the following deliverables at 60% design.

· SWMP Template (Under 1 Acre)

D. Advertisement Construction Documents
Stolfus will address comments from the FOR submittal for a PS&E submittal. It is assumed that the
Town of Avon will do a final review of this set prior to preparation of the Advertisement set. Comments
will be received via Bluebeam session. A review meeting is not planned. Once comments are received
from the PS&E submittal, the plan package will be finalized for Advertisement and an Advertisement set
will be issued.
Stolfus will assist the Town of Avon in answering any Contractor questions that may arise out of the
project advertisement. Stolfus will complete up to two revisions under AD that happen on plans and
specifications.
Deliverables
The Deliverables will include:
• PS&E Plan Package
• Advertisement/Record Plan Set
• Miscellaneous Advertisement and Bid Assistance
E. Field Survey and ROW (Farnsworth Group)
ROW information will be gathered from information available via Eagle County GIS.
Farnsworth Group will provide survey for the project. Scope of work is as follows:
1. Use an existing control network for the site (a modified state plane network):
a. Check the existing survey control monumentation in the area for movement. Set additional
control monuments and aerial panel points where needed.

b. Provide a Project Control Diagram (PDF) showing the control points and their pertinent survey
information for future design and construction use.
2. Provide a topographic survey 10 curb ramps in 3 locations along Avon Road. The survey will include
all permanent features from back of sidewalk to back of sidewalk in areas shown on the Survey Limits
exhibit and as listed below:
a. Area 1 – a 140-foot by 60-foot area on the south side of the intersection of Riverfront and E.
Hurd Lane and Avon Road.
b. Area 2 - a 130-foot by 60-foot area on the south side of the roundabout at Benchmark Rd and
Avon Road. This area includes four curb ramps.
c. Area 3 - a 150-foot by 60-foot area on the south side of the roundabout at E. Beaver Creek Blvd
and Avon Road. This area includes four curb ramps.
NOTE: These surveys will include all topographic features and above ground utilities but do not include
underground utility locating.
3. Provide a topographic drawing with terrain models for each area (6 areas) in an Open Roads format.

The Deliverables will include:

SCHEDULE AND ESTIMATED FEE
We are available to begin work on this amendment immediately upon Notice to Proceed. We will
provide the services described above on a Time and Materials basis at our Standard Hourly Rates with
a “Not to Exceed” limit which we will not charge beyond without prior written approval. Stolfus has
estimated that the above scope of services could be completed for a “Not to Exceed” budget of
$55,095.

Invoices for professional services rendered and expenses incurred will be prepared monthly. Invoices
will include a detailed description of meetings attended, work in progress, and tasks completed.
Payment is expected within 30 days. After 30 days, unpaid invoices are subject to a 1.5% service
charge, compounded monthly.
Limitation of Liability
In recognition of the relative risks and benefits of the project to the Town of Avon and Stolfus &
Associates, Inc, the risks have been allocated such that Town of Avon agrees, to the fullest extent
permitted by law, to limit the liability of Stolfus & Associates, Inc. to Town of Avon for any and all claims,
losses, costs, damages of any nature whatsoever or claims expenses from any cause or causes,
including attorneys’ fees and costs and expert-witness fees and costs, so that the total aggregate
liability of Stolfus & Associates, Inc. to Town of Avon shall not exceed the total compensation received
by Stolfus & Associates, Inc. under this Agreement. It is intended that this limitation apply to any and all
liability or cause of action however alleged or arising, unless otherwise prohibited by law.
