On-Call Agreement for Professional Services
TASK ORDER 2.8
US6 Mobility Improvement Project: Design Services During Construction
This Task Order pertains to the On-Call Agreement by and between the Town of Avon
(“TOWN”), and Stolfus and Associates (“CONTRACTOR”), dated On-Call Contract Execution
of September 3, 2021 (“the Agreement”).
CONTRACTOR shall perform services as provided herein and in the Agreement. This Task
Order shall not be binding until it has been properly signed by both parties. Upon
execution, this Task Order shall supplement the Agreement as it pertains to the project
described below.
Project Description: Provide final design for the US6 Multimodal Safety and Mobility
Improvements project. The project received the CDOT Revitalizing Main Street Grant to
slow prevailing speed on US6 and potentially install 3 pedestrian crossings as shown on the
project site below. Additional funds are needed to design a stormwater drainage system.

Dear Eva,
The Avon US 6 Mobility project is located on US 6 in the Town of Avon. The project starts at
River Edge Apartments (MP 169.5) and ends at Eagle-Vail Rd (MP 171.0). The Town of Avon
identified a need to address safety and pedestrian mobility concerns on US 6 due to the lack of
pedestrian crossings in the project area. The identified need will be addressed through this
project by providing traffic calming through the corridor to facilitate the addition of multimodal
safety improvements, including pedestrian crossings at transit stops. Through a previous contract
with Stolfus & Associates, Inc. (Stolfus), final design for the corridor was completed.
Stolfus is pleased to provide services for the Avon US 6 Mobility construction project. The
following services are included in the scope:
• Design Services During Construction
The following describes the proposed scope of services necessary to accomplish the above task:
SCOPE OF SERVICES
Project Administration/Management
Stolfus will review and process invoices monthly and provide other project administration/
management services (coordination and product review) throughout the duration of the project.
Stolfus will be responsible for day-to-day management and coordination of the work elements.
This will include coordination on Requests for Information (RFIs), plan revisions, and submittal
reviews. Celeste Shinn will be the primary point of contact throughout construction.
Roadway and Traffic Design Services During Construction (Stolfus)
Construction Meetings
It is assumed that Stolfus will attend the following meetings:
• Construction team meetings – assume 1 hour meeting per month for up to eight (8)
meetings (virtual)
• Pre-Construction meeting – one (1) (virtual)
Requests for Information
Stolfus will review and respond to the Contractor’s requests for information (RFIs). All RFIs must
be submitted by the Contractor in writing. Stolfus will review and respond in writing within one
week after receiving a written RFI. Up to five (5) RFIs are included in this scope of services
related to roadway and traffic elements.

Construction Conflict Resolution and Plan Revisions
Stolfus will prepare design plan revisions for conflicts that occur during construction, including
unknown or changed conditions which require additional analysis, design, and/or plan revisions.
Stolfus will provide and seal the revised plans, as necessary and requested by the Town. This
scope of work includes:
• Plan Revisions – up to three (3) sheets
• Specification Revisions – up to two (2) specifications
Advertisement Support
Stolfus will assist with responding to Contractor’s questions during Advertisement for up to four
(4) hours related to roadway and traffic elements.
Drainage Design Services During Construction (RESPEC)
Post Design Services
Post-design services will include reviewing and responding to contractor RFIs, reviewing shop
drawings, bid support, and construction conflict resolution and plan revisions. RESPEC
anticipates reviewing approximately 5-6 materials submittals based on the AD-level design.
Design changes will be shown on revised plans as necessary. RESPEC will be responsible for
revisions, submittals, RFIs, and bid support for drainage items pertaining to the HYDR and
SWMP sheets of the AD set only. Services up to 54 hours for a total of $7500 are included in
this scope of work.
Landscaping and Irrigation Design Services During Construction (DHM)
Post Design Services
DHM will provide construction period services throughout the completion of construction.
Services included: support during bidding, reviewing landscape and irrigation product submittals,
assist with reviewing and responding to Requests for Information (RFI’s), and minor plan
revisions as requested. Services up to $5000 are included in this scope of work.
Lighting Design Services During Construction (Clanton & Associates)
Post Design Services
• Up to four (4) team meetings (virtual)
• Up to two (2) minor plan revisions
• Review lighting equipment submittals (up to two (2) reviews included)
• Review electrical equipment submittals (up to two (2) reviews included)
• Respond to RFIs, up to four (4) hours or two (2) RFIs
• Respond to bidder questions
Survey and ROW Design Services During Construction (Farnsworth)
Field Survey Work
a. Check existing control points for damage or movement
i. It is assumed that the control is in good condition and no other points will need
to be set.
b. Set up to 8 permanent easement monuments for PE-1 and PE-2.
Right-of-Way (ROW) Services:
a. Revise the existing right-of-way plans noting the type of monument set and adding the
recording information to the Property Tabulation sheets.

b. Create mylar copies of the revised ROW plans.
i. Fill out Eagle County Land survey plat deposit form
c. Deposit the mylar plans at the Eagle County Engineering Department.
Two trips are assumed. One for setting the monuments and one for depositing the plans.

Schedule and Estimated Fee
We are available to begin work on this project immediately upon Notice to Proceed. We will
provide the services described above on a Time and Materials basis at our Standard Hourly Rates
with a “Not to Exceed” limit which we will not charge beyond with prior written approval. Stolfus
has estimated that the above scope of services could be completed for a “Not to Exceed” budget
of $40,000 with the following breakdown by discipline:
Stolfus (Roadway/Traffic/PM): $14,700
Respec (Drainage/SWMP): $ 7,500
DHM (Landscaping/Irrigation): $ 5,000
Clanton (Lighting): $ 5,000
Farnsworth (Survey/ROW): $ 7,800
Total: $40,000
Invoices for professional services rendered and expenses incurred will be prepared monthly.
Invoices will include a detailed description of meetings attended, work in progress, and tasks
completed.
