25.08.28 SCA Baker Builders, LLC CO-5 Form Approved by Town Attorney – 05/15/2024 CHANGE ORDER Order No.: 005 Date: August 28, 2025 Agreement Date: November 25, 2024 NAME OF PROJECT: East Nottingham Park Improvements OWNER: Town of Avon, Colorado CONTRACTOR: __Baker Builders, LLC dba Baker Constructors The following changes are hereby made to the CONTRACT DOCUMENTS : Justification: Changes detailed herein capture additional debris encountered at the site, additional water fountains added to the project, building foundation welding, and additional plumbing details and coordination that needed to be added to the Project as well as removal and changes to the Landscaping Scope of the Project. See the spreadsheet in Attachment A for a summary of the revised plan quantities, resulting in a total contract adjustment of $4,269.17. None of adjustments herein result in a change to the contract time. DEMO, REMOVAL, & DISPOSAL ($547): As detailed in Baker PCO_014 and in the picture in, Attachment B, two extra loads of trash and debris had to be hauled from the site due to the substantial amount of buried construction debris found at the site (i.e, piping, concrete, rebar, etc.). This increases the contract by $547 for proper disposal of the debris. LINE PAINT STRIPING (-$1,792.14): Baker requested the Town of Avon to complete the paint striping on the new Southern (aka Eastern) portion of the parking lot. Avon agreed to stripe (18) eighteen 9 by 18-foot parking spaces, or line qty 15 - 18LF, and (24) twenty-four 9 by 21-foot parking spaces, or line qty 23 – 21LF, as illustrated in Attachment C, totaling 753 LF of Line Item 37 Striping (4” White). At $2.38 per LF for 753, this reduces the Contract by $1,792.14. This Line Item 37 was also reduced in CO#3 by 312 LF, Leaving 943 LF of this item in the Contract. ELECTRICAL SECONDARY-SPLICE CREDIT (-$12,403): Secondary Electrical Power was scoped in the contract to repull the electrical power to stage in its entirety. Working with the Contractor and Avon Facilities, it was agreed that a fused splice is robust enough to be an acceptable solution and prevents the extended downtime repull and reconnect the entire service given the high number of bends in the conduit. Baker PCO 015, Attachment D, details the credit associated with this streamlined scope. SITE FURNISHINGS ($14,661): As detailed in Baker PCO_010 Attachment E, two (2) WB761220BK Global Industrial Outdoor Bi-Level Drinking Fountain with Bottle Filler, Black (no dog bowl) were added to the plaza area of the project. This scope includes purchase and complete installation of the fountain pedestals including plumbing connections to the water and CO-5 Form Approved by Town Attorney – 05/15/2024 sewer drains. This adds Line Item 83a: Bi-level Water Fountain with Bottle Filler (including connection) at $7,330.40/EA, adding $14,661 to the contract amount PLUMBING SCOPE ($32,303): Plumbing for the buildings is provided as part of the prefabricated modular. A water service tie-in was indicated in the civil drawings. However, a meter and backflow prevention setup was an oversight that was missed in coordination between the two scopes. Plumbing Drawings (P1.0 & P2.0) were issued on 6/13/2025 to detail these assemblies and connections along with the water service and drain lines for the water stations discussed in the above item. Baker PCO 012, Attachment F, details the cost associated with the installation of items issued in the Plumbing Drawings at $32,303. LANDSCAPING (-$35,866.69): Landscaping design has since been modified to remove areas of plantings to accommodate the waterwise gardening group in the NW corner of the project as well as reducing plantings that will get removed with the pending pedestrian mall project. Additionally, the large landscaping area has been upgraded to sod for quicker restoration. These changes include: - Line Item 66 Deciduous Shrubs: Delete 34 for -$39,712 (Reduced in CO#1 by qty. 6, leaving qty. 11 in the Contract) - Line Item 67 Coniferous Shrubs: Delete 3 for -$3,285 - Line Item 68 Ornamental Grasses: Delete 51 for -$1,861.50 (*per unit price is $36.50) - Line Item 69 Perennials: Delete 347 for -$7,599.30 - Line Item 74a Replace 22,034 sq ft of seeded area with Fescue Sod: Add $16,592.00 - Totaling: a credit of $35,866.69 Marked-up Drawing (L2.1) was provided to Baker on 8/6/2025 to detail the plantings deletions and minor adjustments to the irrigation plan. Baker PCO 017, Attachment G details the increased cost, of $16,592.00, of exchanging the currently contracted seed mix for fescue sod. The irrigation system and ground cover plantings will remain the same, except the drip irrigation for the planting will be substituted for turf irrigation in the NE corner of the sodded area and deleted in the landscape island in the NE corner of the project. FOUNDATIONS – MODULAR WELDING ($7,320.00): During bidding, the structure was anticipated to be continuously bearing along the foundation per the plans. During the design of the modular building, the need to weld the buildings onto the foundation as well as level them arose. This required the design to be modified to have a welded shim attachment every 10 feet. Baker PCO 019, Attachment H, details the cost associated with adding the foundation welding scope at $7,320.00. SITE TRAILER POWER (-$500.00): Baker Requested to plug their ground level trailer into an existing Avon-owned power outlet at the E Park Site. Avon agreed to a flat rate cost of $100/month, Attachment I, for a currently agreed upon credit of $500 for the period of July 18th through Nov 18th. Change to CONTRACT PRICE: $ 4,269.17 Original CONTRACT PRICE: $ 3,940,836.00 Current CONTRACT PRICE adjusted by previous CHANGE ORDER $ 4,028,348.59 CO-5 Form Approved by Town Attorney – 05/15/2024 The CONTRACT PRICE due to this CHANGE ORDER will be increased by $4,269.17.
