CO-4 Form Approved by Town Attorney – 05/15/2024 CHANGE ORDER Order No.: 004 Date: August 13, 2025 Agreement Date: November 25, 2024 NAME OF PROJECT: East Nottingham Park Improvements OWNER: Town of Avon, Colorado CONTRACTOR: __Baker Builders, LLC dba Baker Constructors The following changes are hereby made to the CONTRACT DOCUMENTS : Justification: Avon wishes to replace the water fountains and associate infrastructure at four (4) Town of Avon Park locations. Baker Constructors provided proposal PCO 011, Attachment A, for this scope. This includes the scope for Metcalf Cabin, Nottingham Playground, Wildridge Pock Park, and O’Neal Spur Park Water Fountains Replacements as detailed in the Attachment B design drawings by Yarnell Civil, dated 9/13/2023. However, the water fountains model specified at all four parks will instead be the Global Industrial™ Outdoor Bi-Level Drinking (WB761221BK). Due to change in the water fountain model, the Contractor shall coordinate with the Engineer to ensure the fountain is offset on the concrete pad to allow for adequate ADA access. This change order includes a plumbing permit, with waiver of Town of Avon permit fee. See the spreadsheet in Attachment C for an itemized summary of the revised plan quantities, resulting in a total contract adjustment of $88,727. No change to the contract time is required for this scope. Change to CONTRACT PRICE: $ $88,727.00 Original CONTRACT PRICE: $ 3,940,836.00 Current CONTRACT PRICE adjusted by previous CHANGE ORDER $ 3,939,621.59 The CONTRACT PRICE due to this CHANGE ORDER will be increased by $88,727.00.
