The following changes are hereby made to the CONTRACT DOCUMENTS :
Justification: Changes detailed herein capture over-excavation and rework resulting from deeper
utilities and irrigation main conflict of the Primary Electric Line under Lake Street, the Fire
Hydrant Relocation (Sheet C-04 issued 5/5/2025), and removal of the Moveable Bike Racks from
the project. See the spreadsheet in Attachment A for a summary of the revised plan quantities,
resulting in a total contract adjustment of $24,777 and no change to the contract time.
UTILITIES ($30,472): This item includes the site work changes to cap and relocate the fire hydrant
to the south side of the Recreation Path (Sheet C-04, issued 5.5.25) for these design changes. As
detailed in Baker PCO 002 & PCO 003. See Sheet and Proposals in Attachment B.
- Fire Hydrant (Baker PCO 002): Cap the existing Fire Hydrant connection and cut in a new
location south of the Rec Path adds $7,855 to Line Item 46.
- Electrical Primary Over-excavation & Trench Rework (Baker PCO 003): During the
installation of Vault V1 in Lake Street, it was discovered that the existing underground electric
line was 9-feet below grade instead of the standard ~4-feet and almost directly below the Town
of Avon’s raw water irrigation main. Therefore, it was decided that repulling the electrical wires
from SW266 back to Vault V2 and eliminating Vault V1 would be quicker and more cost-
effective than cutting open more of Lake Street to relocate the irrigation main. Baker’s
subcontractor RPM Construction was impacted by the additional depth of excavation, backfill,
and have to rework 32 LF of conduit in the trench between the V1 and V2 location to meet the
9’depth at the former location of Vault V1. Additionally, Avon requested the two 4” empty
conduits discovered running across Lake Street be extended the width of the excavation prior
to repaving. This over-excavation scope adds $22,617 to Baker’s contract as Line Item 94a.
Note: Baker agreed not to charge Bonds and Insurance fees until the contract amount exceeds
the original contract amount. Holy Cross Energy will provide credit for restocking Vault V1
but will have additional fees for their crews to complete the work.
SITE FURNISHINGS (-$5,695): Line Item 82 Moveable Bike Racks are to be removed from the
Project, crediting back $5,695.
OVERHEAD ELECTRIC BURY ($0): During the installation of Vault V3, it was determined that

the vault installation over the Heat Recovery Piping may be problematic in the future. Therefore,
Vault V3 was relocated further east to eliminate this concern. See Attachment C for details and
approval from Holy Cross Energy and Avon. This change reconciles the outstanding Primary
Trenching w/ Conduit between the railroad casing and the surface location of conduit North of the
Railroad, which was detailed as Outstanding in CO1. There is no price change to the contract from
this scope. Line Item 92 Electric Primary Trenching w/ Conduit between the railroad casing and
the surface location of conduit to the South of the Railroad has not been determined and will be
adjusted via future Change Order once that scope has commenced.
Change to CONTRACT PRICE: $ 24,777.00
Original CONTRACT PRICE: $ 3,940,836.00
Current CONTRACT PRICE adjusted by previous CHANGE ORDER $ 3,865,912.25
The CONTRACT PRICE due to this CHANGE ORDER will be increased by $ 24,777.00.
The new CONTRACT PRICE including this CHANGE ORDER will be $ 3,890,689.25.
Change to CONTRACT TIME:
The CONTRACT TIME will be (increased)(decreased) by 0 calendar days.
The date for completion of all work will be May 20, 2026.

Attached is a proposed CO for the work that Tri-Phase/RPM has/is doing in Lake Street to remedy the deep Primary
intersection.
For the calcs I am assuming the following:
South Vault Elevation:
I think they are down about 2’ below expected so I have an added 2’ of Fill . The overall footprint of the excavation
was roughly 10’ x 10’ and the vault takes up roughly 5’5 of that space.
Robie thinks his crew was there about 4 extra hours for this work
Deep Primary intersection.
I have 16 hours total which is probably a little light but should cover the cost.
Monday the 12th the set the barricades and got the excavation down to about 8’ vs 4’
Tuesday morning they re-called located to make sure they were digging in the right location
Based on the location, they set the trench box and found the sleeve, they spent about ½ of a day widening the
trench for the trench box and hand digging below the irrigation main
Monday the 19th the removed and reinstalled the primary from 32’ back in the trench, Spent the entire day
prepping for Holy cross
Tuesday the 20th, assisted as necessary for Holy Cross
Today, spend most of the day backfilling the deeper part of the trench
Tomorrow, back to scope work for the 32’ of replacement
Friday will be clean up and prep for pavement
They had two trench boxes on site that I saw.
Structural fill calc:
9-4d x 5w x 32lf = 800/2 = 400 CF + 2 extra feet wide for the upper end of the trench (5’ wide vs 3’ wide) so 2’w x 4’d
x 32 LF = 256
656 CF/27 = 24.3 CY x 1.65 to convert to tons = 40 Tons

They did have to spend an extra day potholing all the different utility crossings coming out of the railroad casing,
but there will not be a cost impact other than any easement filings for yourself.
Speaking of easements, does ERWSD have an easement for the manhole that would allow us to work in that area?
