On-Call Agreement for Professional Services
TASK ORDER 1-2023-1
Track G Plat Amendment
This Task Order pertains to the On-Call Agreement by and between the Town of Avon
(“TOWN”), and Stolfus and Associates (“CONTRACTOR”), dated On-Call Contract Execution
of September 3, 2021 (“the Agreement”).
CONTRACTOR shall perform services as provided herein and in the Agreement. This Task
Order shall not be binding until it has been properly signed by both parties. Upon
execution, this Task Order shall supplement the Agreement as it pertains to the project
described below.
Project Description: Provide a plat amendment of Tract G Sections 11 & 12 shown on the
attached. The plat amendment will correct the southerly tract boundary that was
established based upon an incorrect ROW boundary for the UPRR. See Attachment A

Dear Eva:
The Town of Avon has identified a need for a plat amendment of Tract G Sections 11 & 12 shown on the
attached. The plat amendment will correct the southerly tract boundary that was established based upon an
incorrect ROW boundary for the UPRR.
This task order identifies the scope of work necessary to perform boundary research, title review & plat
development, coordination with the Town and County, set monuments and deposit survey.
SCOPE OF WORK
Tasks 1 through 4 – Surveying Scope of Services (SurvWest, LLC)
The attached Survey Proposal provides a summary of surveying scope of services.
Tasks 1 through 4 Deliverables:
•Amended plat deposited with the County
•Setting up to ten monuments and property pins.
Work Element 5 – Project Administration / Review (Stolfus & Associates, Inc.)
SurvWest, LLC will be responsible for the bulk of the work effort for this task order. The role of Stolfus &
Associates, Inc. will be to review and process invoices and provide project administration services. Stolfus will
also conduct a review of the survey deliverables prior to transmittal of those deliverables to the Town of Avon.
Work Element 5 Deliverables:
•Monthly invoices and progress reports.
EXCLUSIONS
•As noted in the attached survey proposal
SCHEDULE
Weather permitting, survey field activities can typically begin within 2 weeks of notice-to-proceed. SurvWest can
provide a Preliminary Plat for review within 4 weeks from Notice to Proceed. Redlines and comments can be
addressed and returned within 2 weeks of receipt.

As detailed below, Stolfus and its subconsultants will perform these engineering services for a total fee of
$20,630.

Assumptions
SurvWest will order necessary title commitments from a land title company. SurvWest will provide an amended plat only for Tract P and
portions of Tract G in sections 11 and 12 as shown highlighted in yellow on the attached Project Location map. SurvWest assumes
setting up to ten (10) monuments and property pins. This scope does NOT include any construction services, construction staking, or
subsurface utility engineering.
